The instructions and checklist below gives information about all the forms and documents needed to complete enrollment and registration of your child at Christian Life Academy. If you have questions, contact the school through email office@clacademy.org, or by phone (309) 449-3346.
First be sure you have read and understand our Enrollment information, Tuition and Payment Plan Options.
Returning families, simply complete and return the Re-Enrollment form. Your registration information is on file with the school and will be verified, updated and completed at registration days in August. Turn in your $50 registration fee ($75 after March 18) with the re-enrollment form.
New families, please make contact with the school before beginning any enrollment paperwork. After your initial visit, when you have made your decision to enroll at Christian Life Academy, use the checklist below to work on registration documents
After completing the preliminary enrollment form, please be sure that you have completed and submitted all of the following:
-- $50 Registration fee (one fee per family registers all children)
After March 18, Registration fee increases to $75 per family

-- A copy of each child's birth certificate
-- Health Documents -- your health care providers should have these forms.
Physical Exam (entering Kindergarten and 6th grade)
Immunizations
Dental Exam (entering K)
Eye Exam (entering K)
